The early signs of bearing failure would be machinery that can run erratically, loudly or with low accuracy.

Monitoring bearing condition with an ultrasound device is the only way to know when a bearing needs lubrication. Bearing grease degrades over time and increases friction levels which are detectable with ultrasound.

Some equipment requires all bearings re greased every day and some once a week, some once a week every two weeks and some once in a month.
