One technique that helps ensure bearings are getting lubricated correctly is acoustic lubrication, more commonly known as ultrasound guided greasing.
The early signs of bearing failure would be machinery that can run erratically, loudly or with low accuracy.
Monitoring bearing condition with an ultrasound device is the only way to know when a bearing needs lubrication. Bearing grease degrades over time and increases friction levels which are detectable with ultrasound.
Some equipment requires all bearings re greased every day and some once a week, some once a week every two weeks and some once in a month.
Never over or under lubricate bearings. The most common errors committed in industrial applications involve adding either too little or too much lubricant.
